Standard Waiver Process
- Physician Evaluation: Secure a signed certificate or DMV application form from a licensed physician (typically a dermatologist or ophthalmologist) detailing the condition and necessity of UV/light-blocking film.
- State DMV Submission: Submit the completed application package to the state's DMV or Department of Public Safety (DPS). Some states only require keeping the physician's signed form inside the glovebox.
- Sticker Verification: In states requiring physical checks, visit a registered inspection station to verify VLT compliance and apply decal indicators.
